I authorize Arrow Construction Resources to investigate my previous background in connection with its consideration of my application. Through this document, or a copy, I authorize any person, agency, institution, company or other entity to give Arrow Construction Resources any and all information they may have, or in connection with the information they receive. I further release from liability any individual, including, including records custodians, for damages that may result to me due to compliance or any attempts to comply with this authorization. I consent to undergo a physical and a drug test, as an applicant or employee, upon request and at the discretion of Arrow Construction Resources and consent to have the results of those test disclosed to Arrow Construction Resources. I understand my application for employment will be denied or my employment will be terminated, if upon request I refuse to submit to testing or refuse to consent to have the results disclosed or discussed, or, if there is a confirmed positive test result. If employed with Arrow Construction Resources, I understand that I would be obligated to abide by all rules, regulations, policies and procedures. In consideration of my employment, I further acknowledge and agree that my employment and compensation may be terminated, with or without cause, and with or without notice, at any time at the option of either myself or Arrow Construction Resources.
